Grade 1-8 Application Packet

Thank you for your interest in our school. We are happy to provide our application packet on-line for download. However, at this time, you cannot submit your application on-line.

Please Note: The due date for the initial round of applications was on February 10, 2010, and this is the date noted in the application cover letter; however, we are still accepting applications for qualified students.

The following Grade 1-8 Application PDF contains all of the application which must be returned to our school by February 10, 2010. In our Grade 1-8 Application PDF, please find the following information:

• Grade 1-8 Application Cover Letter
• Application Form
• School Recommendation Form
• Authorization for Release of Student Records
• Testing Schedule

Please give the School Recommendation Form from the application PDF to your child's current teacher. Your child's teacher must send the Teacher Evaluation to our school. The Teacher Evaluation is due by Friday, February 19, 2010. We cannot accept an evaluation from a parent. Our school address is below:

• A non-refundable $50.00 application fee. Please make the check payable to St. Joseph of Cupertino School. Please do not send cash.
• A copy of your child's baptismal certificate. Please do not send the original document.
• A copy of your child's most recent report card.
